Due to our continued growth, we are looking for a highly organised and energetic Service Coordinator to join our team in Wodonga. Job Description

In this exciting role you will have the opportunity to further develop your skills and knowledge in the NDIS sector. This role will receive ongoing training and support to establish and maintain a client portfolio. You will assume responsibility for the delivery of services to our NDIS clients.  The role will focus on coordinating person centred supports and helping clients to reach their goals and maintain their independence.

Key Responsibilities:

  • Provide excellent customer service to staff, participants, and their personal networks.
  • Liaise and negotiate with relevant service providers/networks/community and referral agencies to ensure the continuity of care for clients and client goals are achieved.
  • Establish, maintain and document service care plans, managing referrals and reviews as necessary.
  • Monitor service delivery, develop and implement change strategies to improve the quality and effectiveness of services provided to clients.
  • Ensure accurate, up-to-date and appropriate case files, records and notes are maintained in accordance with policies and procedures.
  • Ensure progress towards identified goals and objectives are monitored and documented, ensuring feedback is provided regularly to the participant, family and/or any other approved representative.
  • Building, coordinating and rostering a team of direct support professionals committed to the delivery of quality support around each individual matching skill is met as per their requests and ensure continuity of services.
  • Ensure all established administrative processes are followed to assist the smooth processing of requests for services including client records including support plans, service quotes, service agreements.
  • Provide day-to-day direction and coordination of services.
  • Monitor and coordinate services within client case files, service agreements, staff rostering, incidents, complaints and conduct service reviews as scheduled and requested.
  • Maintain an up-to-date knowledge of procedures, guidelines, and statutory requirements relevant to the organisation and its clients.

Desired Skills and Experience

As the successful applicant, you will demonstrate a genuine commitment to high quality outcomes for NDIS participants, carers, and the organisation with a focus on growing our organisation.

For success you are:

  • Experienced in NDIS and hold health or disability sector knowledge, with a minimum Cert IV in disability or another relevant field.
  • A competent person with exceptional customer service skills and a service-oriented mindset.
  • A collaborative team player who excels in working effectively with others.
  • Skilled at navigating various systems and proficient in Microsoft Suite.
  • Results driven with a demonstrated ability to empower others.
  • A demonstrated critical thinker and collaborative worker.

 

Screening requirements: 

  • Right to Work in Australia.
  • NDIS Worker Screening Check, or willing to obtain.
  • Current Drivers Licence and comprehensive car insurance.
  • Working with Children Check.
  • First Aid & CPR.
